If you’re a recipient of the Kaiser Permanente Community Health Program (CHCP), it's time to renew your subsidy! Applications must be submitted by October 1, 2024, to ensure you maintain your benefits through the end of the year. If you don't reapply by this deadline, your subsidy will expire on December 31, 2024, and you will be responsible for full monthly premiums and out-of-pocket costs starting in 2025.
Here’s what you need to do:
1. Complete and sign the reapplication form for all members of your household.
2. Gather proof of income and any other required documents.
3. Submit your completed form and documents by email, mail, or fax before October 1, 2024.
